Sr LPN - Trainer

ESSENTIAL RESPONSIBILITIES:
Activities include but are not limited to the following:
Oversee the daily shift operations.
Provide daily nursing care to include: order medications, provide updates to the RN and practitioners, AMAP orientation and training, schedule appointments, participate in Agency activities, nursing meetings and complete required documentation.
Supervises the safety and well being of the individuals including the recovery and implementation of the treatment plan.
Develop and maintain an effective communication network with the individuals, program staff, families, community agencies and other agency departments.
Serve as a role model, promote the individuals independence by implementing the IPP, and attend to personal self-care and other program needs that the individuals can not independently accomplish.
Maintain accurate purchasing records, petty cash and personal funds, establish effective contact with vendors.
Monitor operation and condition of the building and supplies. Ensure building security and upkeep of maintenance equipment.
Provide input in the development of program activities and ensure the accomplishment of individuals goals and behavior plans, ensure that the individual program plan is implemented and documented.
Maintain site staffing schedule and coverage.
Problem-solve daily shift issues and/or conflicts as part of site leadership team.
Provide coaching and counseling of direct support staff and LPN's to enhance individual and team performance.
Provide input into evaluation of direct support staff and LPN's in collaboration with Program Manager.
Arrange and assist with onsite orientation and training activities for new employees and existing staff.
Assist with interviews for job openings as requested.
Participate in activities as part of the treatment team such as meetings, trainings and committees.
Maintain the timely completion of all necessary records and reports.
Establish a work environment that creates positive communication between supervisors and employees and assimilates new employees to the Agency's culture, values and mission.
Ensure compliance with all pertinent government and agency regulations and operating standards
QUALIFICATIONS/EDUCATION/EXPERIENCE
High School Diploma or equivalent .
Valid NYS Licensure as a Licensed Practical Nurse and current NYS Registration Certificate.
Minimum of one year of experience in nursing and supervisory role.
Ability to lift and/or assist individuals in your care and the ability to meet the physical requirements of the specific job.
Basic Computer Skills (Windows, Outlook, Adobe Acrobat, Word, Excel).
This position has regular and substantial unsupervised or unrestricted physical contact with individuals receiving services and is required by law to be fingerprinted for a criminal history record check under OPWDD (Office for People with Developmental Disabilities).
Must have a valid New York State Driver's License.
Must possess the ability to make independent decisions when circumstances are warranted.
WORK ENVIRONMENT/HAZARDS
OSHA Exposure Category I.
Job related tasks that involve exposure to blood, body fluids, tissue or the potential for skin or mucous membrane contact from spills or splashes of these substances. The use of appropriate protective measures should be required for any employee engaged in Category I tasks.
PHYSICAL DEMANDS
Must have full sight and hearing with fluency in the English language.
Must be able to lift 60 lbs.
Must be able to sit for long periods of time. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position without compromising work-flow and efficiency.
WORK CONTACT GROUP
All staff, individuals at sites, visitors, family members, vendors, various regulatory and professional agencies, independent physicians, nurse practitioners and physician assistants.

SUPERVISES: Residence Counselors, LPNs.


Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
